Superficial Parotidectomy

A superficial parotidectomy is a surgical procedure to remove part of the parotid gland, a large salivary gland located on the side of the face, in front of and below the ear. It is often performed when a tumor or other growth is present in the superficial part of the parotid gland, which is the outer portion of the gland.
The parotid gland produces saliva, which helps in digestion and keeps the mouth moist. Tumors in the parotid gland are often benign (non-cancerous) but can sometimes be malignant (cancerous).

Signs & Symptoms

Signs and symptoms prompting a superficial parotidectomy often include:

  • Swelling near the jaw or below the ear: is typically painless but can become tender if infected or inflamed.
  • Lumps in the cheek or jawline: These lumps might be hard or soft, often non-painful but sometimes growing over time.
  • Facial nerve weakness: In rare cases, a tumor may press on the facial nerve, leading to slight weakness in facial movement on the affected side.
  • Pain around the ear or jaw: This can indicate inflammation, infection, or, in rare cases, malignancy.
Causes

Conditions that might lead to a superficial parotidectomy include:

  • Benign tumors: The most common cause is benign tumors, such as pleomorphic adenomas or Warthin tumors, which can grow within the parotid gland.
  • Malignant tumors: Rarely, malignant tumors develop in the gland, which may need removal.
  • Chronic infections: Repeated infections or abscesses in the parotid gland could necessitate surgery.
  • Cysts or stones: These may obstruct the salivary duct, leading to chronic swelling and discomfort.
When to seek medical attention?

Seek medical attention if you experience any of the following symptoms:

  • Persistent swelling or lump near the ear or jaw.
  • Pain around the ear or jaw area.
  • Facial muscle weakness or numbness.
  • Redness and warmth around the area of swelling (indicative of infection).
Diagnostics & Tests

To confirm the need for a superficial parotidectomy, doctors may order:

  • Ultrasound or CT/MRI scan: To visualize the structure and check for tumors or stones.
  • Fine Needle Aspiration Biopsy (FNAB): A small tissue sample is taken to examine for benign or malignant cells.
  • Sialography: A specialized imaging test to examine the salivary ducts can help locate blockages or stones.
Do’s & Don’ts

 Do:

  • Keep the area clean and dry.
  • Follow up with scheduled post-op appointments.
  • Take prescribed antibiotics or pain medications as directed.
  • Eat soft foods initially to avoid straining the jaw area.

 Don't:

  • Avoid strenuous activities until your doctor approves.
  • Don't touch or pull at the dressing or sutures.
  • Avoid smoking, which can delay healing.

Frequently Asked Questions For Superficial Parotidectomy

Yes, it is a major surgery requiring general anesthesia, but it's often minimally invasive and safe with proper care.

Benign tumors are unlikely to recur if entirely removed. Malignant tumors may need additional treatments like radiation, depending on their nature.

Complications are rare but include facial nerve injury, infection, or Frey's syndrome (sweating while eating).

The incision is usually placed near natural creases or behind the ear, minimizing visible scarring.

Most people return to normal activities within 2-4 weeks, though complete healing may take longer.
