Medical Treatments

Laryngectomy

A laryngectomy is a major surgical procedure in which the larynx (voice box) is removed, typically to treat cancer, severe injury, or irreversible damage to the larynx. The larynx is located in the throat and plays a crucial role in breathing, speaking, and swallowing. When the larynx is removed, breathing and speaking are permanently altered, and a new airway called a stoma is created in the neck.

Signs & Symptoms

Patients undergoing surgery related to cancer may experience a range of symptoms before and after the procedure:

Before surgery (if related to a condition like cancer):

  • Difficulty swallowing or breathing.
  • Persistent hoarseness or loss of voice.
  • Coughing up blood.
  • A lump in the neck.
  • Unexplained weight loss.

After surgery:

  • Loss of voice (as the vocal cords are removed).
  • A permanent stoma (an opening in the neck for breathing).

Causes

Laryngectomy is primarily performed due to:

  • Laryngeal cancer: Advanced cancers affecting the larynx, particularly in individuals with a history of smoking or heavy alcohol use.
  • Severe trauma or injury: Physical trauma, such as car accidents or blunt force injuries to the throat.
  • Radiation damage: Long-term effects of radiation therapy to the head or neck can damage the larynx, sometimes requiring surgical intervention.
  • Laryngeal infections: Rarely, infections or other illnesses leading to significant laryngeal damage may necessitate a laryngectomy.
When to seek medical attention?

If you notice any of the signs and symptoms above, especially persistent voice changes, painful swallowing, or difficulty breathing, contact a healthcare provider. Early medical attention can help with diagnosis and treatment options, potentially avoiding the need for a laryngectomy.

Diagnostics & Tests

Several tests help diagnose conditions that may lead to a laryngectomy:

  1. Laryngoscopy: A scope with a camera is inserted into the throat to examine the larynx.
  2. Biopsy: A small sample of tissue from the larynx is taken to detect cancer cells.
  3. Imaging tests:
    • CT or MRI scans provide detailed images of the larynx to assess the tumor's size and spread.
    • PET scans help in identifying cancer spread to other parts of the body.
  4. Blood tests: While not diagnostic, they can indicate overall health before surgery.
Do’s & Don’ts

Do’s:

  • Follow-up regularly: Attend all follow-up appointments with your healthcare provider.
  • Learn alternative communication: Explore options like voice prostheses or speech therapy.
  • Practice good stoma care: To prevent infections, keep the tracheostomy stoma clean.
  • Stay hydrated and eat a balanced diet: To promote healing and maintain strength.
  • Avoid exposure to irritants: Such as dust, smoke, and strong fumes, as they can irritate the stoma.

 Don’ts:

  • Don’t smoke or drink alcohol: These can delay healing and increase the risk of recurrence.
  • Avoid water near the stoma: Use a stoma guard while bathing or in the rain to prevent water entry.
  • Don’t neglect oral hygiene: Maintain good oral care, as it affects overall health and stoma cleanliness.
  • Avoid strenuous activities: Give your body time to recover and avoid lifting heavy objects.

Frequently Asked Questions For Laryngectomy

Yes, though not through your vocal cords. You may use speech therapy or devices like a voice prosthesis.

While there is no surefire prevention, you can reduce risk factors by avoiding smoking and excessive alcohol, eating a healthy diet, and using protective gear if exposed to toxins at work.

Recovery can take several weeks to months. Physical recovery from surgery often takes about 6-8 weeks, but adjusting to new speech methods may take longer.

You will breathe through a stoma, a hole created in your neck, instead of through your nose or mouth.

There will be discomfort after surgery, but pain can be managed with medication.
