Medical Treatments

Fistulectomy

Fistulectomy is a type of surgery done to remove a fistula completely. A fistula is an abnormal tunnel-like connection between two body parts, like between the anus and the skin around it. When other treatments don't work, or the fistula is complex, a fistulectomy is recommended to remove the entire fistula and its surrounding tissue.

Signs & Symptoms

If you have a fistula, you might experience the following symptoms:

  • Pain and discomfort in the affected area, often near the anus.
  • Swelling, redness, or irritation around the fistula.
  • Discharge of pus or blood from the opening of the fistula.
  • Recurring infections or abscesses.
  • Difficulty passing stool or discomfort when sitting or during bowel movements.
  • Fever if the infection worsens.
Causes

 Fistulas usually develop due to:

  • Chronic infections or abscesses in the affected area, such as anal abscesses.
  • Inflammatory bowel diseases like Crohn's disease or ulcerative colitis.
  • Past surgeries or injuries that leave the area prone to infection.
  • Infections like tuberculosis or sexually transmitted infections that damage tissues.
When to seek medical attention?

If you experience any of these symptoms, you should see a doctor:

  • Persistent pain or discharge from the fistula.
  • Frequent infections or abscesses.
  • Fever or feeling generally unwell.
  • If less invasive treatments like antibiotics or seton placement are not working.

Diagnostics & Tests

To confirm a fistula and plan the surgery, doctors may use:

  • Physical examination: Check the area for any visible signs.
  • MRI or CT scans: To see the full extent of the fistula and its branches.
  • Fistulography: A dye test to map out the fistula on an X-ray.
  • Endoscopy or colonoscopy: To check for underlying conditions, especially in the bowel.
Do’s & Don’ts

 Do's:

  • Keep the area clean: Follow your doctor's advice for wound cleaning.
  • Eat a high-fiber diet: This helps prevent constipation, which could pressure the wound.
  • Take prescribed medications: Especially pain relievers and antibiotics, to prevent infection.
  • Stay hydrated: Drinking enough water can help with digestion and healing.
  • Follow up with your doctor: Regular check-ups are essential for monitoring recovery.

 Don'ts:

  • Don't strain during bowel movements: This can damage the healing area.
  • Avoid sitting for long periods: This puts pressure on the wound.
  • Don't engage in strenuous activities: Avoid heavy lifting or intense exercise until your doctor says it's safe.
  • Avoid delaying treatment: Contact your doctor immediately if you notice signs of infection or worsening symptoms.

Frequently Asked Questions For Fistulectomy

Recovery usually takes about 4-6 weeks. You may experience discomfort for the first couple of weeks, but pain management and rest will help.

While fistulectomy is usually adequate, there is a slight chance that the fistula can recur, especially if you have an underlying condition like Crohn's disease.

The main complications include infection, delayed healing, and, very rarely, incontinence (difficulty controlling bowel movements). However, most people recover well without any long-term problems.

Keeping the area clean, managing conditions like Crohn's disease, and getting prompt infection treatment can help prevent fistulas from forming.

During surgery, you won't feel pain due to anesthesia. After surgery, there may be discomfort, but your doctor will prescribe painkillers to help manage it.
