Electrophysiological Study (EPS)

An Electrophysiological Study (EPS) is a specialized test that examines the heart's electrical activity to assess abnormal heart rhythms (arrhythmias).
A healthcare provider uses this information to diagnose and treat abnormal heart rhythms. This invasive test uses catheters entering your heart to capture signals. In some cases, providers can fix the problem during the EP study.

Signs & Symptoms

 EPS is usually recommended when a patient shows symptoms of arrhythmias., such as:

  • Fainting (syncope) or near-fainting spells.
  • Severe shortness of breath, even at rest.
  • Persistent chest pain.
  • Fatigue and weakness.
  • Rapid or irregular heartbeat.
  • Dizziness or fainting.
  • Chest discomfort or pain.
Causes

 EPS is usually performed to diagnose arrhythmias caused by:

  • Structural heart disease (e.g., heart attack, heart failure, valve disease).
  • Congenital heart defects.
  • Electrolyte imbalances (e.g., abnormal potassium or magnesium levels).
  • Cardiomyopathy (disease of the heart muscle).
  • Abnormal conduction pathways (such as in Wolff-Parkinson-White syndrome).
  • Drug or alcohol abuse.
When to seek medical attention?

 Seek immediate medical attention if you experience any of the following symptoms:

  • Persistent or recurrent palpitations.
  • Unexplained fainting or near-fainting episodes.
  • Sudden, severe chest pain or discomfort.
  • Shortness of breath or fatigue during normal activities.
  • If you have a known heart condition and arrhythmia worsens or doesn’t respond to medication.
     
Diagnostics & Tests

 Various diagnostic tools are available to assess heart health and detect potential issues, including:

  • Electrocardiogram (ECG/EKG): Measures the heart's electrical activity.
  • Holter monitor: A wearable device that records heart rhythms over 24-48 hours.
  • Echocardiogram: An ultrasound of the heart to check for structural issues.
  • Stress test: Monitors the heart's activity during physical exertion.
  • Blood tests: To check for electrolyte imbalances or other contributing factors.

 Do’s 

  • Follow all pre-procedure instructions, including fasting and stopping certain medications.
  • Arrange for someone to drive you home after the procedure.
  • Rest for 24-48 hours after the procedure and avoid strenuous activities.
  • Take medications as prescribed.
  • Keep the insertion site clean and dry.


 Don’ts

  • Avoid heavy lifting or vigorous physical activity for at least a week after EPS.
  • Don’t take non-prescribed medications, especially blood thinners or aspirin, unless approved by your doctor.
  • Don’t ignore new symptoms like excessive bleeding, swelling, or infection at the insertion site.

Frequently Asked Questions For Electrophysiological Study (EPS)

EPS is generally not painful, but patients may feel some discomfort or pressure during catheter insertion. Local anesthesia and sometimes sedation are used to minimize discomfort.

EPS is considered a low-risk procedure, but as with any invasive test, there are risks, including bleeding, infection, blood vessel damage, or arrhythmias induced during the test.

If an arrhythmia is detected, doctors may either continue to monitor it or treat it with ablation or medications or recommend further procedures (e.g., pacemaker).

EPS can help diagnose arrhythmias and guide treatment. If catheter ablation is performed during the test, it may cure the arrhythmia.

Most people can resume normal activities within a week. Light activities can often be resumed after 1-2 days, but strenuous activities should be avoided for at least a week.
