Total Hip Replacement

Total Hip Replacement (THR), or total hip arthroplasty, is a procedure in which a surgeon replaces the damaged parts of the hip joint with artificial components, usually made of metal, ceramic, or durable plastic. This surgery is often recommended for patients experiencing severe pain, stiffness, or limited mobility caused by conditions such as osteoarthritis, rheumatoid arthritis, or traumatic injuries like hip fractures. During THR, the damaged femoral head is removed and replaced with a metal or ceramic ball attached to a stem that fits into the femur. The damaged cartilage and bone in the hip socket are also removed and replaced with a metal socket, which may be lined with a plastic or ceramic spacer to facilitate smooth joint movement. This hip joint replacement helps reduce pain, improve mobility, and enhance the quality of life for patients facing daily activity challenges. THR is typically considered when non-surgical treatments, such as medication or physical therapy, have proven ineffective. Most patients experience significant improvements in joint function and pain relief after surgery, though a recovery period and rehabilitation are crucial for achieving the best results.

Signs & Symptoms

The common symptoms that lead to considering a hip replacement include:

Persistent hip pain, especially when walking or standing

Stiffness in the hip joint that limits the range of motion

Difficulty performing daily activities, such as walking or bending

Pain that continues even during rest, particularly at night

Limping or an abnormal gait

Causes

Hip replacement is often a solution to chronic hip problems caused by:

Osteoarthritis: Wear and tear of the hip joint cartilage

Rheumatoid arthritis: Inflammation damages joint tissues

Hip fractures: Injuries to the hip from falls or accidents

Avascular necrosis: Reduced blood flow to the hip, leading to bone death

Other conditions: Congenital hip dysplasia, bone tumours, or infections

When to seek medical attention?

You should consider seeking medical advice if you experience:

Severe, unmanageable hip pain despite medication and physical therapy

Inability to bear weight on the affected leg

Swelling, redness, or warmth around the hip joint

Restricted movement, impacting daily activities

A physician will evaluate these symptoms and may recommend total hip replacement if less invasive treatments have not provided relief.

Risk Factors

Certain factors increase the likelihood of needing a hip replacement, such as:

Age: Common in individuals over 60

Genetics: Family history of hip-related issues or arthritis

Obesity: Extra weight can wear down hip joints faster

High-impact activities: Sports or work that puts stress on joints

Previous hip injuries: Prior fractures or trauma to the hip area

Diagnostics & Tests

To determine the extent of hip joint damage, doctors use several diagnostic tests:

X-rays show joint space narrowing, bone damage, and deformities

MRI/CT scans: Provide detailed images of soft tissues, ligaments, and cartilage

Blood tests: Help detect underlying conditions like rheumatoid arthritis

These tests help assess the condition of the hip joint and inform treatment planning.

Do’s & Don’ts

 Do's:

Follow your physical therapy plan to regain strength.

Use walking aids as needed to avoid putting stress on the hip.

Take prescribed medications to manage pain and prevent infections.

Sit in chairs with armrests to help you stand up more quickly.

Gradually increase activities like walking, but avoid overexertion.

 Don'ts:

Avoid high-impact activities like running or jumping.

Don't sit in low chairs or on soft surfaces that make it hard to stand up.

Refrain from crossing your legs or bending your hip beyond 90 degrees.

Avoid twisting your torso, which can put pressure on the new hip.

Don't lift heavy objects or engage in strenuous activities until fully healed.
